Enrollment Requirements
The following information is required for enrollment at Pride & Joy Learning Center, Inc.:
The Green Information Sheet which is the State of Florida #1 Card including password. The back side must be notarized by both parents in both columns.
Alternate Nutrition Form must be signed. Lunch is brought from home. We provide morning and afternoon snacks with drinks.
Discipline Policy must be read and signed showing you support our policy.
“Know Your Child Care Center” form must be read and signed, acknowledging you have read the form.
DH Form 3040. Health examination form from your doctor.
DH Form 680. Immunization records including the chicken pox vaccine.
Diapers and Pull-ups: K2 children do not need to be potty trained when they start school. We will assist in training after the training begins at home. Diapers, pull-ups, wipes, ointment, and gloves will be provided by Pride & Joy Learning Center. Parents will be billed for diapers/pull-up cards used during each month.
Child’s lunch box, if staying past 11:30am, must be marked on the outside with the child’s name. Include drink, napkins, fork and spoon. No refrigeration or heating of food can be provided. Pizza Day is one day per week. Apple Juice is also served. Parents are billed monthly if this option is desired.
Provide a change of clothes (shorts, shirts, underwear, socks, and a jacket) to remain at school. Please label each of these items. Wobblers and K2 students will need to bring in two changes of clothing.
Children will not be permitted in school with a skin rash, fever, inflamed eyes, diarrhea, and/or a discolored runny nose. If you give your child any medicine besides an antibiotic, he/she is too sick to come to school. We will only give a child medicine such as antibiotics if the child has been taking them for at least two days. A HRS #5 card must be filled out prior to antibiotics being administered. We have these forms in the school office.
